There should have been a warning prompt about this case when binding the left mouse button, unfortunately it seems that warning doesn't work as it should. We'll look into fixing that asap! "Select and pack new images" means is that it will select and pack only the newly added images. So if you only add one image it won't have anything to pack, but if you add multiple it will lay them out so they don't all end up in the same place.by PureRef - Suggestions
If you're willing to have different shortcuts for your different usages of PureRef then this is possible to do already! From the FAQ: Q: Can I open multiple PureRef instances with different settings?
