Hi, I'm using your software for years, here are my suggestions
Focus image : active the focus on a picture by overing, not selecting it first, if using the space shortcut keyboard
Note : it doesn't behave like the draw mode does. Note stays in place if the picture is rotated/scaled
Hierarchy : allow to open the selected file in an external program of choice

I agree with that, having hidden temp files on hard drive coming from every software installed on a computer that never disappear by themshelves is a big problem!
After all, PureRef is only a viewer, not a maker of pictures, saving a text file with the path of the pictures and a low version should be enough in case of "failure"
You could purge temp files on exit, or add a menu
